WHAT IS BPH?

BPH is an age-related condition that causes prostate cells to enlarge. These glandular cells make the fluid that serves as the vehicle to carry sperm. When a man ejaculates it is 90% prostate fluid and 10% sperm. Unfortunately, when a man is through his reproductive years there is no signal to tell the glandular prostate tissue to quit growing. When the prostate becomes enlarged, the urethra (the tube that carries urine and semen through the penis) is restricted, making urination difficult or painful. Because most men typically don't think about their prostate until they experience a problem, it is viewed as a sensitive and sometimes embarrassing subject. However, BPH is far more common than you may think, affecting approximately 50% of men over age 50 and 90% of men over at 80. With a number of treatments available, you don't have to let BPH control your life. All men worry about prostate cancer but typically when the symptoms have to do with urination, it is much more likely to be due to benign enlargement rather than a cancerous growth. Of course, we always rule out cancer.

WHAT TREATMENTS ARE AVAILABLE FOR BPH?

The most common medications used to treat BPH are Hytrin (Terazosin), Cardura (Doxazosin), Uroxatral, and Flomax. These medications, known as alpha-blockers, relax the muscles in the prostate which relieves urine outflow tension. These drugs work best on small glands, affecting most patients within 24-48 hours. For prostates that are 40 grams or larger in size, Proscar and Avodart are commonly prescribed, which can take from 3 weeks to 2 months to show results. Other treatment options are minimally invasive and can be performed PAIN-FREE in the office under conscious sedation by a Certified Registered Nurse Anesthetist. The two procedures allow for a return to work and daily activities in 4-5 days and sexual activity in 3-4 weeks. When considering surgical prostate treatment, all men are concerned about the side effects of erectile dysfunction and urinary leakage. I find my best results come from radiofrequency heating of the lateral lobes, which is accomplished with the TUNA (Trans-urethral Needle Ablation) procedure. This will shrink the tissue with less than a 1% chance of damaging the nerves that can cause erectile dysfunction. The third, or middle lobe, is then surgically resected using cutting and cautery electrical current. With the combined treatment the chance of success goes from 80% to 95%.

WHAT KIND OF TREATMENT SHOULD I EXPECT FROM THE HUBBARD CLINIC?

On your first office visit, you will receive a prostate exam, urine test, PSA blood test, and a bladder ultrasound to determine bladder retention. If applicable, you will be placed on a two week trial of medication to see if your symptoms improve. The second visit will consist of a cystoscopy and prostate ultrasound to determine the size and degree of prostate blockage. To ease discomfort, you have the option to have this procedure done under local anesthesia and oral pain medication. Pending results, our staff will discuss a detailed treatment plan that will best suit your needs. The type of treatment you will receive depends on the size and degree of your prostate blockage.
